Welcome to Hara kid's clinic
We opened Hara kid's clinic in April 2009 in Nakahetsuki, Oita City, in order to protect the health of local children in a place close to them. We would like to be a "family doctor" that you can come to feel free to consult with anytime about any symptoms that concern you. Together with our staff, we aim to provide prevention, early detection of health problems, and take early countermeasures for the children and their families in the area.

About filling out the medical questionnaire

At Hara kid's clinic, we ask you to fill out a medical questionnaire when you visit our clinic. Please download the medical questionnaire (for first-time patients) from the link below before visiting to the clinic, fill it out, and bring it with you to make the reception process smoother.

About using My Number Card
Our clinic strives to provide high-quality medical care by acquiring and utilizing medical information. Please note that my number card can be used as a conventional health insurance card. If you wish to use your My Number Card to obtain and utilize patient information, please cooperate by agreeing to obtain medical information. If you have a "Children's Medical Expense Subsidy Eligibility Certificate," please be sure to bring it with you when you visit our clinic.
